Music Mix provides professional DJ entertainment for your special occasion
(weddings, graduations, birthdays, anniversary, divorce, holiday parties,
reunions or party of your choosing). To
reserve R&K Music Mix for your event, simply complete and return this form
with a fifty dollar ($50.00) non-refundable deposit.

contract to you. If your form is not
returned to us, your date will be made available to everyone else. Please call if you are unable to honor this
request to make other arrangements. The cost of our service for
your wedding is $ for a 4-hour time frame. Additional charge of $50.00 per hour after
that will apply if applicable. Final
payment is due prior to the end or your evening. Client
agrees to notify R&K Music Mix of any pertinent changes in event location
or cancellation at least 1 week prior to your event date. Client agrees to hold R&K Music Mix
harmless and indemnify R&K Music Mix from any liability, including
reasonable attorney fees, for failing to abide by this agreement. Client
